PURPOSE: The Dexcom G6 Continuous Glucose Monitoring (“CGM”) System (“Dexcom G6”) can be provided free of charge for the sole purpose of allowing a healthcare provider and/or patient to evaluate whether Dexcom G6 is right for the patient. The Dexcom G6 is cleared by the FDA for use with a separate piece of durable medical equipment called a receiver. This pack includes one (1) Dexcom G6 sensor and one (1) transmitter and does not include a dedicated receiver. In the event the patient’s treating healthcare provider, in their independent clinical judgment, determines that the Dexcom receiver is medically necessary for their patient during the limited evaluation period, the receiver can be separately purchased from Dexcom. Accordingly, the treating healthcare provider should exercise their clinical judgment, in consideration of the Dexcom G6 Instructions for Use, to only dispense this pack to a patient who (or whose authorized caregiver) (i) has reliable access to a compatible smart device† and/or automated insulin delivery system, and (ii) is capable of safely using the products in this pack with their smart device, without the need for a separate dedicated receiver. Please note that if the end user of this pack is a Medicare beneficiary and the individual elects to continue using Dexcom G6 after this sample, the individual will need to obtain a medically necessary Dexcom receiver in order for Medicare coverage to be available.

PRESCRIBER AUTHORIZATION REQUIRED: This pack contains medical supplies for which a prescription is required. The items in this pack may only be used at the direction of a licensed prescriber. Authorized prescribers may provide this free pack or items from it to patients if permitted under state law.

This means that, regardless of whether you are a healthcare provider, patient, or caregiver, you must not sell these items, trade them, bill a patient or customer for them, or submit a reimbursement request for them to any third-party payor, such as a government healthcare program or commercial health insurer.

BRIEF SAFETY STATEMENT Failure to use the Dexcom G6 Continuous Glucose Monitoring System (G6) and its components according to the instructions for use provided with your device and available at and to properly consider all indications, contraindications, warnings, precautions, and cautions in those instructions for use may result in you missing a severe hypoglycemia (low blood glucose) or hyperglycemia (high blood glucose) occurrence and/or making a treatment decision that may result in injury. If your glucose alerts and readings from the G6 do not match symptoms or expectations or you’re taking over the recommended maximum dosage amount of 1000mg of acetaminophen every 6 hours, use a blood glucose meter to make diabetes treatment decisions. Seek medical advice and attention when appropriate, including for any medical emergency.
